Comprehensive and Compassionate Case Management

Brigid Collins works with DCYF and self-referrals to accept clients into our Case Management based programs. These sensitive family matters are handled with professionalism, kindness, empathy and on-going support by our team of accomplished case managers.

FAMILY SUPPORT TEAM (FST)

FST is the umbrella service that encompasses advocacy, coordination, communication, and collaboration in support of the individual goals of the client. This voluntary program is based on the “parenting role” including that of a caregiver.

PARENT -CHILD ASSISTANCE PROGRAM (PCAP)

PCAP is an award winning, evidenced-informed home visitation case-management model for pregnant and parenting women with substance use disorders. The program goals are to help mothers build healthy families and prevent future births of children exposed prenatally to alcohol and drugs. Operated by the University of Washington, we are one of many PCAP sites in the state. Clients in PCAP self-refer to the voluntary, 3-year program and we also consider referrals from vetter sources.

MOTHERS IN RECOVERY (MRP)

In the Mothers in Recovery program, we offer intensive case management for mothers struggling with sobriety. The client must either be under substance use treatment or completed in the last year. This intensive 18-month program carries an expectation of weekly contact and the goals are to facilitate communication, coordination and collaboration with their service provider in hopes of will achieving a positive outcome, whatever that may be for each individual client.

KINSHIP NAVIGATION

(Skagit County families only)

Kinship Navigation and Kinship Caregiver Support Programs offer support services for relatives or close family friends who are providing full-time care for children, whether through formal or informal arrangements. This includes grandparents, aunts, uncles, adult siblings, and non-relative extended family members (NREFMs) in Skagit County. Our programs help kinship caregivers navigate legal, financial, and community resources to ensure they have the tools and support they need to thrive.
